Your Rights Regarding Your PHI
You have the following rights with respect to your health information:

Right to Inspect and Copy: You have the right to view and request a copy of your PHI, with limited exceptions. Requests must be submitted in writing. Reasonable fees may apply.

Right to Amend: If you believe the information I have is incorrect or incomplete, you may request an amendment. I may deny your request if the information is accurate or if I did not create the record.

Right to Request Restrictions: You may request limitations on how your PHI is used or shared. I will comply when feasible and in alignment with your safety.

Right to Confidential Communications: You may request that I contact you at a specific phone number, email, or mailing address. I will accommodate all reasonable requests.

Right to Request Deletion: You may request that certain communications or documents be removed or redacted when permissible.

Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with me or with the Secretary of the U.S. Department of Health and Human Services. I will not retaliate.
